After 30,000 pages his black is getting streaky! What has gone wrong? He's also only using one set of cartridges. None of the cartridges have ever been flushed.

Try an OEM cart. Run a service test print. If OK, then just use the new cart for refilling...

Switching to a new cartridge should directly show it.
