So long as you don’t print photos.. The maxify uses BK black only..I've got some ink that was for a Pixma MX870 and I am wondering if I can refill a Maxify MB5120 with it?

Your Pixma MX870 uses only one Pigment ink - the CLI-220 Black. The other color inks of the CLI-221 cartridges (CYM) are Dye-based inks and not for use in the Maxify.
This is unknown as I do not have knowledge of the CLI-220 ink used in a Maxify. It is possible that one of several scenarios may appear from different cooling characteristics, leaking, too much ink flow, or a bizarre reaction between the residual Maxify ink and the CLI-220 ink. If the pigment particle size is larger than what the Maxify is made for then you may get clogging. Keep watch on the prints and stop printing immediately if you see an issue such as loss of ink on the printed page.Should I be concerned about causing damage?

@palombian 's words carry much weight. He has been refilling the Maxify for some time. My words are theoretical. Pal's are practical, earned from experience.
My general thought is it should work as long as the ink is not much different than the Maxify ink. Unless someone with working knowledge comes forward and says not to, I would give it a try if I had a sufficient quantity of CLI-220 ink. But I am not you.
